    • unofficial transcript (only for transfer students grades 9-11)
    • proof of last grade level completed
    • attendance record
    • discipline record
    • standardized test scores
    • reports of any special testing/psychological evaluations, IEP, or 504 (when applicable)
  • Application Fee of $25 (check payable to York Catholic).
